Do we have to replace immediately a lost tooth? What can cause a missing tooth? How we can replace it? Is the dental implant the best solution? What is the difference between the dental implant and the dental bridge?

 

Damaging effects of a missing tooth

There is a deception all around the world in connection with replacing a missing tooth. Most of people think if a tooth is missing, but it is not a visible one, it is not urgent to visit a dentist. The most obvious effect of missing teeth is aesthetic. After the eyes what we see on each other? The smile and the teeth. Living with missing tooth can cause depression for a while, mainly it goes hand in hand with foul breath. The people feel that their environment is avoiding them, and it is unfortunately sometimes can be true.

Beside the aesthetic problems one or more missing tooth can cause also functional problems. We can feel damaging effects in short and long term too.

We have to talk about the chewing surface like one unit. It means we have the upper and lower chewing’s surface instead of we have x pieces of tooth. These surfaces have to be proper because of their integrity.  The correct chewing capacity is given by the chewing’s surfaces from both side which is necessary to have the right quality of life.  The aesthetic is another important question – our missing tooth is visible or not?

With losing a tooth, the continuity of the chewing surface breaks therefore we should think in units, and it is good to replace it as soon as possible.

Our teeth and their pairs

„The teeth are looking for their pairs”- this is a dentist’s joke. It means if a tooth is missing, the pair of the lost tooth at the opposite side is going to remove its place and maybe it would dislocate as well. The bone is going to rarefy and decrease after a while. The adjacent teeth are going to lean into the gap, towards to the direction of the missing tooth. If the adjacent tooth going to lean onto the front, a domino effect will start again. The leaned tooth cause a gap and on the opposite side, it can be a problem later. The more time we wait replacing a missing tooth, the more difficult the implantation can get. Is these cases usually bone grafting and sinus lift are necessary.

The degree of the tooth next to the gap can also change, it means the chewing changes, it won’t be axial anymore, it can result the dislocation of the tooth. If it falls out after a while, the opposite tooth will also fall out. Because of the missing tooth, we cannot chew only on the other side. The chewing joint can be overworked.

Domino effect

This is a long process, it doesn’t build up overnight. Through the overload and the shift of the power system, the joints of the jawbone can be infected. These types of infection’s healing period takes a long time. The joints of the jawbone are twin joints, and the two ends of the bone there are two joints, and if there is a problem on one side, the problem will affect the other side too. On the other hand, we can’t pulp food small enough, what can cause stomach and bowels problems. By the way our teeth have an important role regarding articulation, in this case an impediment in speech can develop too.

Avoiding domino effect, we should replace even one missing tooth. This is a basic role.

Finally, a missing tooth can cause the followings:

  • The healthy teeth will move to the direction of the gap
  • They lean over their roots won’t be stable enough anymore
  • The power of the chewing decreases
  • The gum will be infected
  • Each bit will cause pain, eating will be distressful, we won’t chew the meals enough and it will affect our digestion
  • A tooth has an important role regarding the articulation, so some speech defect can develop too
  • The loadability of the jawbone decreases, it will be thinner and become weaker
  • The joint of the jawbone will be infected
  • The muscles of the face and the neck will be deformed
  • This deformation affects to our backbone and across it to our whole body
  • Our posture, walk and the position of the organs will also change

What is a dental implant?

 

A dental implant is an artificial root which is screwed into the jawbone to replace a missing tooth and consists of 3 different elements:

  • The implant itself which is fixed into the jawbone
  • The pillar which is between the implant and the dental crown
  • The dental crown to replace the visible part of the missing tooth

The implant is made of a hard metal called titanium. This material is fully compatible, which means no allergic reaction is expected and it has the feature to be linked to the living bone in a natural way.

The dental implant is the best solution to replace one or more teeth as it’s esthetical, comfortable and has the functionality of an original tooth. It also provides a reliable and safe solution long term.

Treatments steps on placing dental implants

 

Inserting a dental implant consists of 3 steps and requires 2 trips to Hungary.

 

  1. Pre-implantation medical consultation

(First trip takes 4-5 days)

 

The first visit to the Clinic starts with a medical consultation before the implantation itself with our professional dentists who will examine you and carry out an x-ray in order to:

  • Confirm that the patient’s general condition will not inhibit inserting the dental implants.
  • Examine the quality and thickness of the jawbone to confirm that there is no need for osteoplastic (bone replacement) to insert the dental implants.
  • Check the quality of the gum and clean the oral cavity (treat potential inflammation and remove dental deposit) if necessary.

As a result of these examinations the doctor makes a diagnosis and prepares a personal treatment plan. If the patient accepts the proposed treatment plan, the treatment can be started.

 

  1. Insertion of the dental implant and the temporary insertion of the clamp

(Step two on the first visit)

A dental implant insertion is a simple and quick surgery which isn’t any more complicated than a tooth removal. A dental implantation with local anaesthetic takes approx. 15-30 minutes (a general anaesthetic can also be requested by the patient). During this treatment, the doctor lances the gum and drills a hole in the jawbone in order to insert the dental implant. There are 2 options available.

 

Option one: The implant is covered with the gum and it helps the dental implant to be built in an airtight environment. It takes around 3-4 months and a micro surgery is needed to open the gum and free up the implant in order to have the dental cap put on.

 

Option two: The implant is not fully covered, there is no need for a second surgery to free up the head of the implant and a temporary cap can be put on the implant on the same or the following day. This is called an immediate implant and it is possible if the doctor thinks the bone is in a proper shape for this kind of treatment and the patient also accepts the instructions about careful chewing.
This is the final step of this phase of the treatment.

 

  1. Insertion of the pillars, preparation and insertion of the final crown

(Second trip takes 3-5 days)

 

If the implant is covered during the first visit a small surgery is needed and the gum is cut by laser to free up the implant. This step takes only a few minutes and doesn’t cause any pain due to the local anaesthetic. In case the top of the implant is not covered by the gum the doctor can immediately put the pillar and the crown on the implant. The next step is to proof the teeth and based on the result the final crown will be prepared.

During the last visit to the clinic the doctor will put the final crown onto the pillar. It can cause some pain to the gum but it doesn’t last more than a few seconds.

 

Replacement of one tooth

 

Dental implant

A dental implant is the most appropriate solution to avoid the problems mentioned above. The teeth nearby should not be defaced as the crown is inserted onto an artificial root and pillar.

As the implant is built into the bone it stimulates the bone itself in a natural way. Last but not least, the dental implant is the best solution, thanks to modern ceramic crowns, which feel and look like a real tooth.

Dental bridge

Classical dental bridge is a conventional solution to replace a missing tooth. This is a fixed solution, aesthetic, functional and easy to put in. However, a dental bridge has two main disadvantages:

  • The two teeth nearest to the missing one have to be polished to use them as the pillars of the bridge, regardless of whether they are in a good condition or not.
  • Where the missing tooth is the bone will be drawn back after a while.

Replacement of more teeth

 

Partial, removable denture

 A removable bridge could be a conventional solution to replace more than one missing teeth next to each other. This is a partial removable denture made of acrylic resin which rests on the gum and the remaining teeth are supported by a metal hook. The part being reproduced is made of resin, whilst the artificial teeth are made of ceramic.

 

On the other hand, this solution may cause inconveniences:

  • The size and instability could create discomfort.
  • It can limit chewing functions.
  • It has to be cleaned after every meal.

 

Fixed bridge on 2 dental implants

It’s the perfect solution to eliminate all discomfort caused by removable dentures, In addition, the fixed bridge also ensures the aesthetics and functionality of natural teeth. This treatment consists of the implementation of 2 dental implants which are used as pillars of the ceramic crown. This solution avoids the insertion of an implant for every tooth, even if one of them is missing.

Thus, providing stability and resistance, giving back the ability to eat anything you want. The greatest advantage of this procedure is that the jawbone is flexible.

In some cases, when the jawbone is in a very bad shape and cannot be reproduced, the removable dentures may be the best solution.

Removable denture on 2 or 3 dental implants

 

This solution can help to fix the removable denture better and to decrease instability. It can increase the chewing capacity and also helps the recovery of the wound due to the moving denture. This treatment can also help to avoid the bone being disappeared in order to improve the appearance of the teeth on a long term.

In case the full set of teeth are missing, it is possible to avoid using a removable denture that can cause many inconveniences:

  • Less attractive appearance.
  • Non-fixed denture laying on the gum that does not look nice.

Fixed denture on 5-6 dental implants

 

This method means absolute stability and as the implants completely release the pressure from the gum it should hold only the denture itself. The shape of the denture increases the hygiene as you can brush your teeth just like your original ones. The denture can be removed only by a professional dentist.

  • Periodical adjustments are needed due to the loss of bone.

A very satisfied patient

Since my childhood, I have had serious dental problems (I have already had some decayed tooth, and some of them dislocated) and I spent a lot of time at different kind of dentists, that caused my fear from the dentist’s offices from time to time. After the lack of regular dentist’s check I wasn’t surprised when I found myself without any smile, however I was only 47 years old. I had only a few healthy teeth left, and I thought I can’t postpone my dental treatment to fix my teeth anymore. So, I decided to look for a proper dentist.

Knowing about the time of the dental procedure was here, it was hard to go on like I’m afraid of smiling because of my teeth or rather the lack of it and that is the reason I was ashamed. My wife convinced me to get it done and I started to search for the best dentists and found the final solution. I’m a distrustful and a worried person, I spent a lot of time on finding an unexceptional resolution. A came to Budapest for a week to check the 5 chosen clinic on the Internet, I visited several blogs and read patients’ opinion and feedback regarding the dental implants in Hungary.

I went to 5 different clinics in Budapest and its neighbourhood, and before I went back to Geneva, I also visited the Breier clinic. I saw immediately, this clinic will be the perfect one. The clinic itself and the environment was so modern, quiet, clean. After I discussed everything with Dr Breier, this meeting helped me to make the final decision. The doctor’s calm attitude and his charisma enslaved me. I found what I was looking for: the confidence.

Considering the seriousness of the operation (extraction of 16 tooth, sinus lift, placing of 12 implants). The treatment started at 7 am in the morning and it finished at 12, I have to tell you what I felt from the beginning until the end of the operation, Dr Breier’s precision and accuracy. During the surgery, he found out that one more implant is needed, but it wasn’t included in the quotation, I got it for free. Is it nice, isn’t it?

I would like to highlight the kind backup I got, the assistant explained all steps of the treatment and calmed me down continuously. I spent one week in Budapest (I mean Pilisvörösvár), at the end of the week I got a temporary bridge for my upper and lower jaw. After that I needed 6 mouths healing period, which was important before the final crowns.

I felt the change, the effects of the new teeth after the first visit. I got my lost smile back. I became a new person. If somebody still hasn’t been at the same boot with me yet, couldn’t imagine how I felt. The second visit was almost only a formality. They place the new crowns, but this trip was actually a vacation with a city tour and lots of entertainment. The price of my new teeth (life) cost 18.000 EUR.

I came back to Hungary for a check up on July, 2016 (it’s free), where I met at the clinic one of my friend, who made the big decision for this serious step. He got 11 implants. I would like to thank everything for Dr Breier and Clément (the French assistant, who is responsible for French speaking patients) for their professional competence. How can I say in one word my endless happiness, what you can feel after you find your lost smile again? My visit kept only a week and during this short period on time the amazing and perfect doctors of Breier clinic allowed it to me. The staff, the assistants were well-prepared, professional and calm, the hygiene was perfect, the atmosphere was so calming.

The choice between the clinics wasn’t hard for me at all, last year one of my colleague’s husband got a treatment at Breier’s clinic. He was absolutely satisfied; the liability and professionalism of the clinic was clear for me. Dr Pálóczi made an unbelievable work on me, the result is a smile with 24 teeth. In addition, this result was less than a week. Root canal treatments, extraction of a few teeth, removal of bad roots and crowns and finally two unplanned extractions which were done by a bridge.

I came to Hungary for dental treatment, like everybody from France and Europe, because in my country it is unpayable. I had to come for 3 visits, it took more than a year, and the most amazing is that I have never met this kind of high level treatment and support in France.

Without of any detail of the whole procedure, I got a fully fixed upper denture, with a help of 12 crowns and 3 implants. All together cost 6000 EUR. This kind of procedures would cost three times more in France.

At the beginning, it was so calming to me that the Breier’s clinic proposed the same treatment plan what I have already got from two famous oral specialists in France. After a tooth extraction, sinus lift, more, than 4 months healing period the placing of the implants, finally, 6 months later I’ve got the final crowns placed healing period placing the crowns. Indeed, the process would seem quite long, but the result is guaranteed only if we do everything by the book.

Dr Breier did my treatment from A till Z, his professionalism and knowledge is amazing, the environment at the clinic was similar to a professional dental clinic in Paris, where the price of an implant is 3000 EUR (instead of 1000 EUR in Hungary). The technicians and the doctors are working together. In France, this is not common.

There is a huge advantage of Breier clinic, they do all kind of treatments in one place. I needed 4 root canal treatments and I just had to go to the next surgery room. Dr Pálóczi, the other, very talented implantologist, without any waiting time, started the procedure immediately. The whole system of the clinic is very well-organized and it is for us, for the patients. The dentists work by heart, you can see that on their attitude and the results. Professional job, professional place! Well-oiled team work, very well educated workers. French dentist could take an example. This is not a joke, you can get a BMW for the price of a Lada.
